SPREADSHEET 1: INDIVIDUALIZED INTERVENTION LOG
Documentation
Keep track of all you need to implement, schedule, and document school wide individualized interventions on a single page.
Record grade, name, teacher, active session, requested/established time for intervention, program, etc.
Sort / Filter
Teacher names alphabetically to guide teacher discussions.
Grade levels numerically to guide grade level discussion.
Intervention Program to support scheduling.
Hide
Hide time requested initially by teacher once time has been. established (save printing space without deleting info.
Hide extraneous data (i.e. teacher, interventionist, etc.) when making recommendations.

SPREADSHEET 2: BEHAVIOR INTERVENTION CONTACTS
Documentation
Keep track of behavior supports provided each day.
This is a great resource when meeting with families or teams.
Document behavior support that does not require an office referral.
Analyze patterns in behavior support to determine effectiveness of current interventions.
Sort / Filter
Student names alphabetically to guide team discussions and pattern analysis.
Dates chronologically for weekly or monthly analysis
Type of support or follow up recommended.
Hide
Hide students names if analyzing trend data.

SPREADSHEET 3: ASSESSMENT CERTIFICATION/TRAINING LOG
Documentation
Staff completion of assessment training for Progress Monitoring, Benchmarking, or Diagnostic Assessments.
Document grade, date status confirmed by report, teacher name, specific assessment/ CBM for training, etc.
Record attempts to contact staff to remind them of required trainings.
Sort / Filter
Filter out all COMPLETE to find staff that needs training.
Filter out all inactive groups because they no longer need to be trained.
Grade levels numerically to guide grade level discussion.
Filter out all staff but one to take a screenshot that can be sent in an email if evidence of documentation/contacts is needed.

Hide columns with data irrelevant to the meeting or task at hand.

SPREADSHEET 4: FAMILY COMMUNICATION LOG
Documentation
Communication between intervention coach and families.
Record student, grade, teacher, date, communication type, contact person, etc.
Perfect documentation for meetings with admins, families, or to serve as contacts for other referrals.
Sort / Filter
Filter out all but a single student to print the log for meetings.
Sort/Filter by communication type to easily count each type of communication.

SPREADSHEET 5: TEACHER CHECKLIST
Documentation
Quick way to track of teachers as you complete various tasks (i.e. individual meetings, reviewing data, surveying teachers, printing needs, meeting attendance, etc.).
Sort / Filter
Sort teachers alphabetically or grades numerically.
Filter specific grade levels or grade ranges.
Hide
Hide columns with data from complete tasks. Hiding this allows you to store the data for future reference but keep it out of the way.

Documentation
Observations for between intervention coach and families.

SPREADSHEET 7: INTERVENTION TEAM SCHEDULE
Documentation
Organizes schedule of entire intervention team into a single page.
Allows for quick adjustments in schedules, duty assignments, planning assignments, etc.
Sort / Filter
Not utilized on this spreadsheet.
Hide
Hide columns of intervention team members that are not relevant to the current discussion.
Hide rows to omit times that are not relevant.

SPREADSHEET 8: INTERVENTION COACH MEETING LOG
Documentation
Document Topic, Type, Date, Notes, Follow-up for meetings with teachers, families, interventionists, administrators, etc.
Organizes all meeting information on a single sheet.
Sort / Filter
Sort by teacher or student name, grade, meeting type, etc.
Filter for all meetings on a single student, with a single teacher, or of a certain type.
Hide
Hide extraneous or incomplete columns or rows as needed.

SPREADSHEET 9: INCOMING STUDENT RECORD
Documentation
List all incoming students as they enroll.
Record communication with previous school, enroll dates, contacts, teacher, grade, previous tiers, recommended tiers, etc.
Sort / Filter
Filter out all but a single student to print all info for one student.
Sort to count all incoming students who entered intervention on a specific tier.
Hide
Hide columns with data irrelevant to the meeting or task at hand.

SPREADSHEET 10: PROGRESS MONITORING FIDELITY & SPS LOG
Documentation
Record all information for progress monitoring groups school wide including Name of PM List, Tier, Teachers, active, dates, content area, intervention, etc.
Use this tool when reviewing progress monitoring data for completion and trend analysis.
Record “Up to Date” if all data is complete.
Record trend analysis:
G=Good Progress,
Q=Questionable,
P=Poor.

Filter out all but a single PM list, teacher, or student.
Sort/Filter by active/inactive, up to date data, etc.
Hide
Hide columns with data irrelevant to the meeting or task at hand.

SPREADSHEET 11: TIER 3 STUDENT LIST AND RELEVANT DATA
Documentation
Record all information for available for Tier 3 students.
Note any data missing: screening, teacher, tiers, PM list, PM tool, etc.
Collect needed data and screenings for all Tier 3 students to ensure all information is available when next steps are discussed.
Sort / Filter
Filter out all but a single student to print data for meetings.
Sort/Filter by active tiers, teacher, PM tools, screenings needed, etc.
Hide
Hide columns with data irrelevant to the meeting or task at hand. Great way to hide all data except grade, teacher, student, and screenings needed when communicating with nurse or speech pathologist.




With 11 spreadsheets, there is a structure for almost any data you would need to collect including:
Individual Interventions: keep track of all of the 1-on-1 interventions occurring in your school.
Behavior Interventions: monitor all students that require behavior support but may not need an office referral.
Assessment Certification/Training Log: record CBM, Strategy, or Program training all in one place. This is great evidence that teachers providing interventions are highly trained.
Family Communication Log: Record all family contacts, meetings, and attempts for documentation (may be needed for next steps).
Teacher Checklist: Easy way to monitor completion of a school wide survey, recommendations, schedule changes, CBM trainings etc.
Instructional Fidelity Record: Record all intervention observations with dates, programs, strategies, etc.
Intervention Team Schedule: Full-day schedule for all intervention members on one page. Great to see if an interventionist has space for new groups/tasks or has enough planning/lunch time.
Intervention Coach Meeting Log: Record details about all meetings attended by coach for evidence of attendance (district, SBDM, administrative, teacher/student meetings, etc.
Incoming Student Record: Ensure all data has arrived and/or been collected on incoming students.
Progress Monitoring Fidelity and SPS Log: List and review all Progress Monitoring groups and tools for appropriateness and correctness (i.e. start score, goal, etc.). Once established list, use tool to monitor Progress Monitoring Fidelity and complete a trend analysis in the same process.
Tier 3 Student List and Relevant Data: Record all data on screenings, tiers, start/end dates, etc. on each Tier 3 students. This ensures any missing data (i.e. vision screenings) are completed before a student gets too far in Tier 3 interventions.
